In addition to our rules, all human guests entering The Bark & Sip facility are required to sign our Detailed Liability Waiver and Assumption of Risk Agreement.

A physical copy of the waiver must be signed by every human guest aged 21 or older before entry to The Bark & Sip.

Submit Your Forms & Schedule Your Dog's Assessment

This section guides you through submitting your required forms and scheduling your dog's essential behavioral assessment.

1. Complete Your Online Application:

   • This is where we gather all the necessary information to create your profile.

2. Securely Upload Veterinary Records:

   • During the online application process, you will be prompted to securely upload your dog's current vaccination records.

   • Required Vaccinations: Rabies, Distemper/Parvo, Bordetella, Canine Influenza (H3N2 & H3N8).

   • Required Test: A negative fecal test result within the last six (6) months.

   • Spay/Neuter: All dogs over 6 months of age must be spayed or neutered.

3. Our Team Review:

   • Once you've submitted your application and all required veterinary records, our dedicated team will carefully review your documentation to ensure everything is complete and meets our health and safety standards.

4. Receive Assessment Scheduling:

   • Upon successful approval of your submitted paperwork, you will receive an email from The Bark & Sip with clear, easy-to-follow instructions on how to schedule your dog's mandatory behavioral assessment.

5. Attend the Behavioral Assessment:

   • Bring your dog to The Bark & Sip at your scheduled assessment time. Our trained Canine Coaches will conduct the assessment to ensure your dog is a good fit for our off-leash environment.

   • For Day Pass Visitors: A quick, informal observation will typically happen upon your arrival to ensure immediate suitability for the day's playgroup.
